Pyodermatous skin lesions: A rare case of extra-intestinal manifestation of inflammatory bowel disease

ABSTRACT

We present a rare case of invasive pyoderma gangrenosum (PG) lesions associated with underlying ulcerative colitis. A 23-year-old male developed painful skin lesions with purulent drainage on the left leg, chest, and face. He underwent multiple debridements without improvement. Additional history revealed prior bloody diarrhea. On evaluation, he had extensive painful skin defects and fever. Skin and colon biopsies showed inflammatory ulcerative changes, confirming inflammatory bowel disease with likely PG. Despite suspicion for infection, bacterial cultures were negative. He clinically improved after two weeks of antibiotics without corticosteroids. This complex case highlights the variability in presentation, challenges diagnosing, and managing aggressive extraintestinal manifestations of inflammatory bowel disease.
