Introduction: Strongyloidiasis is a parasitic disease caused by the parasite Strongyloides stercoralis that affects 100 million people worldwide, primarily in tropical and subtropical areas.
Objectives: We report a case of detection of strongyloid larvae in peripheralthe blood of a 65-year-old female patient with a past medical history of prolonged use of steroids.
Case presentation: She was hospitalized at the Center for Tropical Diseases-Bach Mai Hospital wwith chief complaints of fatigue and joint and bone pain, fever and manifestations of respiratory distress. The initial diagnosis was either a severe sepsis with pneumonia or suspected tuberculosis. Patients was treated with empirical broad-spectrum antibiotics but not improved. The patient was then diagnosed confirmedly by chance when was tested for peripheral blood and it was found strongyloid larvae. This is extra-ordinary as all serological tests and also strongyloid larvae in the stool were negative. Treatment of Ivermectin for strongyloidiasis was started immediately right after doagnosed, resulting in improved clinical status and patient was discharged from the hospital.
Conclusions: This is the first case of a bloody larvae in Vietnam discovered and published. The patient was immunocompromised and responded well to the Ivermectin, although the stool and the succus gastricus tests were negative.
