Evaluation of the left ventricular function of patients with severe heart failure before and after implantation of cardial resynchronization (CRT) devices using Speckle tracking echocardiography (STE)

ABSTRACT

Objective: This study aims to evaluate left ventricular function of patients with severe heart failure before and after the implantation of cardial resynchronization therapy (CRT) devices using Speckle tracking echocardiography.

Methods: We conducted a prospective longitudinal study with a convenient sample size of 22 patients with severe heart failure (left ventricular ejection fraction (LVEF) ≤ 35% and QRS duration ≥ 120 msec) who underwent CRT device implantation at Vietnam National Heart Institute - Bach Mai Hospital.

Results: The mean age of patients in our study was 60 years old, and men accounted for 84.8%. The mean baseline QRS duration was 133.9 msec. The QRS duration at the time of discharge was 124.4 msec, compared to 115.6 msec at 1 month and 111.8 msec at 3 months of follow-up. The severity of mitral regurgitation improved while there was a decrease in pulmonary artery systolic pressure after CRT implantation and during the follow-up (both p<0.05). The diastolic left ventricular function improved significantly at 1 month after CRT implantation, with a decrease in the E/e’ ratio and an increase in e’, a’, and s’ velocities (both p<0.05). The left ventricular global longitudinal strain (LVGLS) improved at 1 month after CRT implantation.

Conclusions: Patients undergoing CRT implantation had a narrowing of the QRS duration from 133.9 msec to 111.8 msec over a 3-month follow-up. The severity of mitral regurgitation and the pulmonary artery systolic pressure both decreased after CRT implantation. The statistically significant reduction in E/e’ was observed right after CRT implantation. Meanwhile, e’, a’, and s’ velocities started to experience a statistically significant increase at 1 month after CRT implantation. The left ventricular global longitudinal strain improved markedly after 1 month of CRT implantation.
